The Uber Boss route starts after the credits roll on Uprising. Not during a normal chapter. Not as a side objective you can complete during the story. The campaign and the endgame are separate loops.

Fragment Location Tips
Proving Grounds: After the Agaddon Brute Champion fight, don’t follow the marker to the Henchman. Check the back wall of the arena. There’s a previously locked door that’s now open.
Purgatory: In the ice tunnel, take the right fork instead of the left. The left fork leads to the story path. The right fork dead-ends at a fragment.
Hell’s Core: The second fragment (Nightmare only) is behind the shield unlock mechanism. You need to complete the mini-arena fight, then check the alcove behind the spawn point.

- Kill Warlocks first in Phase 2. Two Warlocks alive = immune boss. This is the most common wipe.
